November 2015 Manual Update

I've gone through and done a major update on all the skills and effects in the manual. Hopefully those sections should be much clearer and easier to understand now. As always I will continue updating other parts of the manual in the future.

for the future, assuming youll keep updating this, i recommend adding the version of MM in which the skills were added. if you just add that each time you edit a description it should barely be any extra work, yet be very useful information
