After installing the texture pack, enter the following command to replace the object on the head with the model.
Remember to take off your good helmet on the head first to avoid direct replacement.

Get the model command

/give @p minecraft:iron_horse_armor{CustomModelData:11}

Put on a hat command

/item replace entity @p armor.head with minecraft:iron_horse_armor{CustomModelData:11}

Put on a hat command (supposedly a bedrock slab)

/item replace entity @p slot.head with minecraft:iron_horse_armor{CustomModelData:11}
